  • One-stop immigration medical exam: Medical history, immunization, TB gold test(blood test), and syphils test 

  • USCIS designated Civil Surgeon

  • Completion of USCIS Form I-693 in sealed envelope.

USCIS "Report of medical examination and vaccination record" is not complicated but necessary step for your green card application. You will be required to show your identification (passport or driver's license) and complete required medical exam with age-appropriated immunization.
